Single Tooth Replacement
Case 1: Dental implant replacement of missing front tooth
Problem: Congenitally missing upper incisor tooth, minimal space for implant.
Solution: 3-D scan analysis and virtual surgery planning for precise implant placement and restoration.

Problem:Heavily diseased upper incisor tooth, visible in smile.
Solution:Gentle atraumatic removal of the tooth and immediate implant replacement.

Problem:Lost premolar tooth due to failed root canal therapy.

Solution:3-D Imaging analysis of the jaw bone to plan precise implant placement in thin area of bone whilst avoiding a graft procedure, subsequent restoration with implant crown.

Multiple Teeth Replacements
Case 8: Dental Implant replacement of 2 lower back missing teeth
ProblemHeavily decayed and infected lower molar teeth.
Solution:Extraction and following a healing period, replacement with dental implants.
Result:

ProblemSeveral missing back teeth, some secondary to failed bridgework resulting in difficulty eating.
Solution: Following 3-D bone analysis and planning, dental implants were able to restore the patient´s bite and chewing.
Result:

ProblemThis patient´s extreme gag reflex and dental phobia had led to major deterioration in his mouth. He suffered from heavy intrinsic tooth staining, difficulty eating, reduced confidence, and difficulty eating. Due to long term missing teeth, the jaw bone had developed advanced loss/shrinkage of bone.
Solution:Following 3-D imaging and virtual surgery planning, in just two comfortable sleep dentistry appointments, the patient´s entire upper teeth were reconstructed and bone grafts were used to prepare the resorbed lower jaw for dental implants. An instant new smile resulted in a happy and confident patient. A few months later, dental implants were used to restore the patient´s full chewing ability and a stable, reconstructed bite.

Full Arch Implant Reconstructions/Full Mouth Implant Reconstructions
ProblemGeneralised decay, failing crowns & root canal therapies, gum disease affecting remaining upper teeth; Dental pain and several missing upper teeth, leading to expansion of the sinus in the upper jaw (maxillary antrum pneumatisation).
Solution:Full clearance of remaining upper teeth and immediate temporary denture replacement. Three months later, 3-D computer guided implant surgery was planned and carried out to the upper jaw under a comfortable Sleep Dentistry (IV Sedation) appointment. Sinus elevation/sinus lift and bone graft procedure was carried out at the same time, allowing new bone formation to support strong implants. The patient continued to wear her denture until the final implant crowns were attached to the implants. She is happy to be biting apples again.

ProblemExtracted lower teeth due to advanced gum disease
Solution:Following advanced 3-D planning of the case, accurate computer guided implant surgery was carried out in the comfort of Sleep Dentistry (IV Sedation). An acrylic set of teeth designed on a titanium framework was fabricated to fit and attach onto the implants. The patient has thrown out the old floppy dentures and now enjoys a solid, fixed, attractive set of lower implant teeth.

ProblemInfected and heavily gum diseased upper teeth not viable for repair.
Solution:Extraction of upper teeth and immediate replacement with temporary dentures. Three months later, 3-D computer guided implant surgery was planned and carried out to the upper jaw under a comfortable Sleep Dentistry (IV Sedation) appointment. The patient continued to wear her denture until the final implant porcelain crowns were fabricated and attached to the implants 3 months later. She now enjoys a healthy mouth, smiling and eating with confidence.
Result:

Problem: Advanced disease to all lower and upper teeth including dental decay, failed restorations and infections. A tremor in his jaw meant denture wear was not feasible for this patient, who also suffered from dental phobia. Due to many missing teeth, this patient´s jaws had also undergone shrinkage which provided an additional challenge for implant placements.
Solution:Following a full clearance of his diseased teeth and saving of a few viable teeth in the upper jaw, 3-D computerised surgery planning allowed very accurate multiple implant placements in both jaws. A sinus elevation and graft was carried out in the upper right jaw to allow placement of a dental implant in that region. Following healing the implants were all restored with 'screw-retained' multi-unit dental bridgework skilfully fabricated to attach to the dental implants. All treatment was carried out under the comfort of IV Sedation/Sleep Dentistry, making it a breeze for the patient who now eats normal foods after decades of suffering from oral health problems.
Result:

Problem: This patient suffered from very advanced gum disease which affected her entire set of teeth, destroying most of the bone around her teeth, leaving them loose and infected. Her fear of dentists had kept her from seeking professional help for decades.
Solution:The first step was to rid the patient of her advanced severe gum disease by removing her teeth and replacing them temporarily with dentures. Since her gum disease had destroyed much of the supporting bone of the upper jaw, bone grafting procedure was performed (with sinus elevation) to create more plentiful bone to support dental implants – all at the same time as her extractions and in the comfort of deeply relaxing Sleep Dentistry (IV Sedation). Following a healing period advanced 3–D computerised planning allowed a full mouth implant reconstruction surgery, to be accurately performed in her mouth. Porcelain teeth were then designed to fit and attach onto her dental implants, recreating a strong functional set of teeth and attractive smile which the patient had missed for decades.

Stabilisation of Loose Dentures With Dental Implants
Case 16: Upper jaw denture stabilisation with dental implants
ProblemLong term upper denture wear leading to advanced bone loss, flabby gums and a very loose denture. Very little gum and bone structure remained for the support of a stable denture. This patient had seen other dentists but had been unable to find any solutions.
Solution:Despite the advanced bone shrinkage due to long term missing teeth, advanced 3-D computerised planning allowed Dr Misagh Habibi to locate 4 specific sites which could support dental implants, and a 3&nash;D computer surgical guide was made to facilitate precise implant placement. Sinus elevation and bone grafts were done at the time of implant placement to create a stronger foundation of bone, and the flabby gum tissue was removed – all in the comfort of Sleep Dentistry (IV Sedation) as the patient snoozed. Following a period of healing, 'Locator' retentive attachments were attached to the implants and inside the denture base, stabilising the denture dramatically. The patient was so used to loose, ineffective dentures that he first wondered if the result was 'too stable'! He is now enjoying a normal diet and dentures that don't move until he tries to remove them.

ProblemThis patient, who suffered from extreme dental phobia, presented with only several lower teeth, which were loose due to advanced gum disease. She had avoided the dentist for years but presented to our practice knowing Dr Misagh Habibi and team are expert at treating nervous patients.
Solution:Under a single comfortable Sleep Dentistry (IV Sedation) appointment, the hopeless teeth were removed and two dental implants placed which then acted as a base to stabilise her lower full denture. The patient is very pleased with her new smile and most importantly, able to eat with confidence knowing the dentures will stay still and stable.

Reconstructive & Cosmetic Non-Implant Dentistry
Case 18: Lower upper jaw dental rehabilitation
ProblemThis patient suffered from years of teeth grinding, leading to advanced tooth wear and a collapsed lower facial profile and unattractive smile.
Solution:A full arch rehabilitation and smile makeover was given to the upper teeth, utilising porcelain crowns. A protective splint/bite guard which the patient wears at night, to make sure any further damage from grinding is prevented. She is over the moon with her new bite and smile.
Result:

ProblemThis patient was unhappy with her crowded and uneven teeth. Additionally, a high smile line and gummy smile compromised the appearance of her smile. Despite her bubbly nature, she was not confident smiling.
Solution:Smile makeover utilising a cosmetic gum lift and re-contouring followed by porcelain veneers. Sleep Dentistry was utilised to make the experience even more pleasant and comfortable. This patient now smiles and speaks with a renewed sense of confidence.
